How to Price Botox: Per Unit vs Per Area (and What to Charge in 2026)

Step 1: Calculate your true cost per unit

Vial cost is not your cost per unit. Account for waste, reconstitution loss, and any units written off for touch-ups. Most spas land between $4.50 and $6.50 true cost per unit after waste — meaningfully higher than the $4 'invoice cost' people quote.

Step 2: Decide per-unit vs per-area

Per-unit pricing is transparent, scales with patient need, and is what most experienced injectors prefer. Per-area pricing is simpler for new patients to understand but can lose money on heavy-dose patients.

A common hybrid: per-unit pricing as the standard, with a flat 'first-time consult + 20 units' intro offer to anchor new patients.

Step 3: Benchmark against your market

Typical 2026 U.S. ranges: $11–$14/unit in mid-tier metros, $14–$18/unit in major metros, $18–$25/unit in luxury markets. If you are below your market average and full, you are leaving money on the table.

Step 4: Build in a clean margin

Target 65–75% gross margin after true cost per unit. At a $5.50 true cost, that means $16–$22 per unit. If your current price doesn't get you there, you have a pricing problem, not a volume problem.

Step 5: Raise prices the right way
